Happy . Hope you are this June!

Week 2:
https://swallowstudy.com/deep-dive-into-esophageal-dysphagia-structure-motility-more/

This infographic shares the bare bones of the 10 question areas that a clinician asks to discover signs of esophageal . Please see my blog for all the details of what to ask and what are the clinical implications.

Comprehensive are essential to help the medical team in the differential diagnosis process.

EXCERPT from blog:
"I have been writing about esophageal dysphagia for years. I am passionate about how much the SLP can contribute to the person’s evaluation and management of their potential esophageal problems. When a person experiences the terrifying sensation of food, liquids, or pills getting stuck, we cannot necessarily divide the issue cleanly: Is it an oropharyngeal problem or an esophageal problem? Swallowing is a continuous, synchronized chain of events with complex throat to esophagus interactions..."

Did you know dysphagia (difficulty swallowing) affects as many as 15 million Americans — and can impact people of all ages? From stroke survivors and people with Parkinson’s disease to head & neck cancer survivors and older adults, swallowing disorders are more common than most realize.

Dysphagia can lead to serious complications including malnutrition, dehydration, aspiration pneumonia, and reduced quality of life.

This Dysphagia Awareness Month, help us raise awareness by sharing this post, starting conversations, and helping others learn the signs and impact of swallowing disorders. 💙

June is National Dysphagia Awareness Month!
💙 WEEK 1: Neurogenic Dysphagia
See this infographic from my blog:
https://swallowstudy.com/understanding-neurogenic-dysphagia-brain-connection/
(Link in Linktree/Bio)

💙 Excerpts:
*** "Because so much of the brain is involved in swallowing, many different neurological conditions and diseases can lead to neurogenic dysphagia (in oropharyngeal and pharyngoesophageal phases). This is a very simplified introduction to 5 common areas representing the underlying causes of neurogenic dysphagia."

*** "Every brain is different. Management must be tailored to the individual’s goals. That is the foundation of person-centered care. Is your goal to enjoy a summer barbeque or your favorite cup of coffee?
The SLP can provide options and ideas for how to compensate with safer swallowing strategies, ...
All these recommendations are tailored to your wishes, needs, and preferences and in line with your goals of care."

*** "The exercises have to follow the principles of neuroplasticity or neural plasticity, such as use-it-or-loose-it and completing enough repetitions to make a difference. The 10 principles of "neural plasticity" were first integrated into dysphagia treatment by a working group (see Robbins et al., 2008). The exercises not only strengthen, but hope to “re-wire” the brain-to-muscle connections."

June is National Dysphagia Awareness Month!
💙 See this infographic for key takeaways from my Intro Blog to my 4-part series this June: https://swallowstudy.com/dysphagia-awareness-month-swallowing-disorders/

💙 Here is an excerpt:

"Dysphagia doesn't discriminate. It affects everyone from premature infants to the elderly. However, the causes of dysphagia define the treatment. Performing a good comprehensive dysphagia evaluation is the first step in treatment, as the medical team needs to figure out what are the underlying causes of this symptom known as dysphagia. Then, the foundation of dysphagia treatment/management is a person-centered care approach. You are at the center of your medical team. You should review findings with your medical team and seek answers to all your questions before making informed decisions about your care."

Enjoyed these courses. I highly recommend by !
and advanced CEU content - terrific every year!

This year’s very brief highlights for me:

You get actual reference values from Dr Steele for your videofluoroscopic swallow studies to know what is really a delay (for example).

Dr Molfenter provides great explanations of sarcopenia, homeostenosis, and how she proved this happens in the pharyngeal muscles. She shares exciting new research on proactive exercises to hold back the muscle breakdown with aging. She shared great Frailty scales too.

The SLP Amber Heape shared crucial skilled nursing facility perspective on person centered care and engaging the whole person in their medical team making informed decisions and “informed refusals!” Just don’t call someone non-compliant. The SLP is not the one to tell someone what they are “allowed” to eat or not. “Dysphagia management is rarely a risk free option.” Balance what may be safer with the person’s wishes and quality of life. This person centered approach and Good documentation and good team conversations can help avoid fears of litigation (as I showed in my Medbridge talk with Dr McCoy!)

Lastly ENT, Dr Abu-Ghanem shared the ENT “workup” for age related normal changes to physiology (primary presbyphagia) versus primary or secondary sarcopenia. Pushing for oropharyngeal dysphagia (beyond that of normal aging) to be considered a “geriatric syndrome.”
