Welcome to Male Excel! The nation's leading TRT & ED provider. Life changing healthcare. Delivered
Dr. Fotinos says one of the best investments you can make in your health in your 40s is getting exercise, nutrition, and sleep right. That's the foundation.
If you're doing all three well and still struggling to build muscle, manage your weight, or sleep well, there can be many different causes. Hormonal factors are one thing a medical provider may consider alongside the rest of your health picture.

Low thyroid function can contribute to symptoms such as fatigue, brain fog, changes in body composition, and difficulty maintaining energy or performance.
It can also affect the hormonal environment that influences testosterone. Thyroid dysfunction may contribute to changes in body composition, including increased visceral fat, and can influence testosterone levels and the proteins involved in testosterone transport.
That is why persistent symptoms should not be evaluated by looking at testosterone alone.
At Male Excel, thyroid function is included as part of a broader clinical evaluation for hormone optimization. Symptoms, medical history, laboratory findings, and the overall clinical picture are considered together to determine whether treatment is appropriate.

Fatigue, low libido, and brain fog are often written off as just getting older. Dr. Fotinos breaks down what a testosterone screening can, and can't, tell you about a man's health, and why the number on a lab report is only part of the picture. A provider can help you figure out what your results actually mean for you.

Chronic stress can affect testosterone production through the interaction between the body's stress and reproductive hormone systems. Dr. Fotinos sees this often in patients who don't think of themselves as "stressed," because they're dealing with illness, injury, or physical strain rather than the mental kind. And stress isn't only psychological. Illness, injury, sleep loss, inadequate recovery and sustained physical strain can also create physiologic stress that affects the hormonal system. When that stress becomes chronic, it can become one of the factors affecting your hormonal health.

Most people know what to expect when taking an ED medication. Few know that a study of more than 29,000 men with ED found tadalafil exposure was associated with a 19% lower rate of major cardiovascular events, 45% lower rate of unstable angina, and 44% lower rate of death from any cause.
You've probably been told a single blood test can confirm whether you need TRT. Dr. Fotinos walks through why that number rarely settles it on its own, free testosterone comes closest as a marker, but even that has limits. What tells him the most is how your body actually responds once treatment starts. At Male Excel, our providers treat you based on both your symptoms and your blood test results to optimize your hormonal health.

Dr. Fotinos has seen firsthand how hormone therapy can help military members and veterans, both while still serving and long after. For those still serving, testosterone can help with performing at the level their teammates depend on. For veterans, many dealing with the after-effects of service often see meaningful improvement once their hormones are addressed.

"He said, I never would have married her if it wasn't for Male Excel." That's what a stranger told our founder Leif at a bar in Key West, tears in his eyes, his new wife standing beside him. Leif says he hears versions of this constantly. Not just about relationships. About promotions, about goals people finally had the energy to chase down. Confidence is the thing that keeps showing up in these stories. Once it's back, everything else starts to feel reachable.
