Empowered Patients, Better Outcomes

When patients understand their health, they make better decisions. Research shows that individuals who actively engage in their care—by asking questions, knowing their treatment options, and following through on care plans—experience fewer complications, faster recoveries, and better overall outcomes.

Yet, too often, poor communication leaves patients —confused, overwhelmed, and uncertain about their next steps.

It's time for healthcare to do better. Clear communication isn’t a luxury; it’s a necessity. When patients have the knowledge and confidence to , they can take charge of their well-being.

Let’s bridge the gap. Let’s turn silent patients into informed advocates for their health. Because when patients understand, outcomes improve.

The Silent Epidemic of Poor Discharge Communication 🚨
Every day, patients leave hospitals with a stack of paperwork, a list of medications, and vague instructions that often leave them confused. Studies show that nearly 40-80% of medical information is forgotten immediately, and half of what is remembered is incorrect.
The result? Poor understanding of discharge instructions is a leading driver of preventable medical errors and hospital readmissions.
📊 Key stats that highlight the crisis:
🔹 Nearly 20% of Medicare patients are readmitted within 30 days of discharge, costing the healthcare system $26 billion annually (CMS).
🔹 One in five discharged patients experiences an adverse event, with 66% of those being medication-related (JAMA).
🔹 Up to 50% of hospital readmissions are linked to inadequate communication during discharge (Agency for Healthcare Research and Quality).

💡 What can we do?
✔️ Implement simpler, clearer discharge instructions
✔️ Use technology-driven solutions to reinforce key points
✔️ Empower patients with interactive education and real-time support
✔️ Improve provider-patient communication to close the loop
A patient who doesn’t fully understand their discharge plan is at higher risk for complications, readmission, and long-term health issues.

🚨 HOT TAKE: Patients Don’t Need to Be Involved in Their Own Healthcare Decisions 🚨

Let’s be honest—who has time to explain things to patients? Shared decision-making? Second opinions? Informed consent? That’s all just red tape slowing us down.

Doctors know best. We spent years in training, so obviously, we should make all the decisions. Patients should just nod, smile, and trust the process. Why burden them with options when we can make their choices for them?

Want to review treatment risks? Too much information is scary.

Thinking about asking questions? Google already confused them enough.

Considering a different approach? We don’t have time for hypotheticals.

If anything, keeping patients in the dark is a sign of efficiency. Decisions get made faster, and we avoid all that pesky “empowerment” talk. After all, why collaborate when we can dictate?

But then again…Maybe that’s why they don’t follow through with treatment. Maybe that’s why healthcare feels broken.

What if—wild idea—we actually help patients engage in their care? What if we shifted from “Doctor Knows Best” to “Doctor and Patient Know Together”?

That’s exactly what we’re tackling with Raport LLC. Patients shouldn’t feel hashtag in their own care. They should have real-time clarity—without needing a medical degree. It’s time to break the silence and hashtag .

Now, let’s hear from you: What’s the worst case of “doctor knows best” that you’ve seen? Drop it in the comments. ⬇️ hashtag hashtag

Repeat after me: Healthcare is about patients. Health care is about patients. Healthcare is about patients.

Yet, every day, patients feel unheard. They leave appointments confused. They hesitate to ask questions. They walk away from healthcare encounters with more uncertainty than clarity.

This isn’t just a problem. It’s a crisis.

At Raport, we believe communication is the foundation of good healthcare. Without it, even the best treatments, the most advanced technology, and the most skilled providers fall short.

Patients deserve to be understood. They deserve clear explanations, shared decision-making, and a voice in their own care.

At ViVE 2025, we’re seeing groundbreaking innovations in AI, digital health, and automation—technology that has the power to transform healthcare. But without better communication, even the most advanced solutions risk missing the mark.

It’s time to break the cycle of miscommunication, frustration, and silence. It’s time to empower patients and providers alike with tools that foster clarity, connection, and confidence.

Because healthcare is—and always will be—about patients.
