On the 25th anniversary of September 11th, we remember everyone who was lost and everyone who answered.

Firefighters, police officers, paramedics, nurses, doctors, and volunteers gave everything to help strangers. For many, the health consequences of that day are still being lived with now.

We're grateful for their service, and we hold them in our thoughts today.

Autonomous Coding Accuracy Starts With the Clinical Note 08/20/2026

Most autonomous coding evaluations focus on accuracy rates, specialty coverage, and EHR integration. Those are absolutely worth asking. But an upstream question often gets skipped.

An autonomous coding engine reads a clinical note. A complete, clinically specific note gives the engine what it needs to perform. A sparse or reconstructed one forces it to work from diminished input, producing code assignments that are defensible but not necessarily optimal.

Documentation quality, not the engine itself, sets the accuracy ceiling in autonomous coding.

For most of the last decade, clinicians have been serving the EMR.
Documenting on the clock. Typing while the patient talks. Spending as much time at the keyboard as at the bedside. The notes got done, but the encounter was compromised in the process.

The shift that ambient AI makes isn't just about efficiency. It's structural.
Our VP of Product & AI put it directly in a recent conversation: "The EMR becomes a service to them versus them serving the EMR." That reframe matters. When documentation happens around the encounter instead of during it, the clinical relationship changes. Physicians practice the way they were trained to practice. Patients get a provider who is present.

InstaNote was built on that premise, that the technology should disappear into the background of the encounter, not compete with it for the clinician's attention.
