But if your patient has already tried them and their progress has plateaued, you need to be able to assess what’s actually limiting them and choose what to do next.
That’s the difference between having a list of treatments and having the clinical reasoning to adapt your treatment to the patient in front of you.

You can memorize that certain movements are restricted after a flexor tendon repair.
OR you can understand the anatomy well enough to know WHY.
If you know where the tendon travels and which joints it crosses, you can start reasoning through what could put stress on that repair.
That’s a much stronger way to study than trying to remember a long list of disconnected precautions.

Before you start memorizing flexor tendon injuries and protocols for the CHT exam, make sure you understand the anatomy underneath them.
Can you identify P1, P2, and P3?
Do you understand the difference between intrinsic and extrinsic muscles?
Do you know where the flexor tendons insert?
Because once you understand the anatomy, it becomes much easier to build the injuries, treatments, and clinical reasoning on top of it.

Following a flexor tendon protocol is one thing.
Understanding WHY you’re doing each step is another.
When you understand the nature of the tissue and what you’re protecting, the progression starts to make more sense.
That’s what I want therapists to build—not just the ability to follow a protocol, but the clinical reasoning to understand what they’re doing and why.

Memorizing radial nerve innervations is one thing.
Knowing what those innervations LOOK like in a patient is another.
A higher injury can affect more than one isolated movement. You have to think through what’s happening above and below the injury and connect that back to the anatomy.
That’s where studying starts becoming clinical reasoning.
Instead of only asking, “What does this nerve innervate?” start asking, “What would I actually SEE if this nerve were injured here?”
That’s how I want you approaching the material inside my CHT Exam Prep Program.

Sometimes the technique isn’t the problem—the POSITION is.
A shoulder movement that hurts your patient in sitting may feel completely different in supine.
Why? You’ve changed how they’re moving, how much they have to actively control, and how gravity is affecting the movement.
So instead of forcing something that hurts, I want you thinking:
Can I change their position?
Can I make this more passive?
Can I approach the same structure differently?
That adaptability is a huge part of hands-on treatment.

The orthosis you make today may NOT be the orthosis your patient needs four weeks from now.
For this patient with a P2 fracture, I made a hand-based ulnar gutter orthosis while her pins are in.
But once those pins come out and she’s allowed to move more?
Maybe we get rid of the orthosis, maybe we modify it, maybe she needs something different based on where she is at that point.
That’s the clinical reasoning behind splinting that I want therapists to understand—not just how to heat up material and fabricate something.

If your patient has a radius fracture and you’re ONLY paying attention to the wrist, you may be missing a much bigger picture.
Structures can matter when you’re trying to improve supination, pronation, wrist motion, and pain.
