State Departments of Health are deciding how Rural Health Transformation Program dollars get allocated right now. For EMS agencies, this is less about winning funding and more about becoming a resource the state can build with as these programs take shape.
A few things worth knowing if you're starting that conversation now:
๐ Bring outcomes, not just requests. States are weighing tradeoffs across chronic disease management, workforce development, and care delivery. An agency that arrives with clear data (response times, treatment-in-place rates, etc.) gives the state something concrete to work with. A request with no evidence behind it is easy to set aside.
๐ Map your work to what the state has already committed to. Minnesota built its RHTP application around a Treatment in Place (TIP) model its agencies had already adopted, rather than introducing something new. That let the state lean on existing data sources for reporting, and let agencies fold into a program built around work they were already doing. Checking your state's stated RHTP priorities before proposing anything ensures youโre aligned with its goals.
๐ Watch how dispatch and coordination are shifting. Hawaiโi is consolidating EMS dispatch under its Department of Health, a project that aims to reconcile county-run systems, fire department partnerships, and state contractors across different islands. EMS agencies tracking that kind of restructuring early can be prepared to adapt.

Register early. Most states require agencies to be properly registered in their procurement systems before they can apply for or accept funding. It's a small administrative step, but missing it can stall a proposal that's otherwise ready to be approved.
Most states are still distributing RHTP funds through open grant programs. Find the person managing that process at your Department of Health, and put a proposal in front of them before the next round closes.
