Foundation for Health Equity supports communities in Northwest Philadelphia by building on the assets

In 2021, Green Tree Community Health Foundation became Foundation for Health Equity, renewed its commitment to addressing issues of health equity, and concentrated its grantmaking in neighborhoods of Northwest Philadelphia facing the greatest barriers to optimal health. These include neighborhoods in the 19118, 19119, 19128, 19138, 19144, 19150 zip codes. We provide direct support to organizations fulfilling immediate, individual needs as well as addressing historical, systemic inequities. For example, we fund programs that feed hungry families as well as work to end food deserts. We also fund programs that help older adults age in place as well as protect their rights. Our goal is to improve systems that have historically made it difficult for people to gain and maintain health.

Big changes are coming to Medicaid! Starting January 1, 2027, some Medicaid recipients in PA will need to meet work requirements and will need to have their Medicaid eligibility checked every six months instead of every year.
