Nobody wakes up on their 40th birthday and decides:

“I think I’m going to become weaker, gain 20 pounds, stop doing athletic things and lower every expectation I have for my body!”

It happens a lot more quietly than that.

You stop doing one thing because your knee bothers you.
Work gets busier.
Training becomes inconsistent.
You gain a little weight.
You stop playing the sport.

Another year goes by.

And eventually the guy who used to have a high physical standard starts saying things like:

“I’m just getting older.”

Of course your body changes as you age.

But I think we’ve confused aging with accepting decline.

I’m far more interested in a different question:

What if your 40s weren’t about desperately holding onto the guy you were at 25?

What if you built someone better?

Stronger.
Leaner.
More capable.
More disciplined.

And still athletic enough to actually USE the body you’ve built.

You don’t need your old glory days back.

You need a new standard worth chasing.

Follow Dr. Jonny Monteiro PT, DPT | Performance Physical Therapist to learn how you can get back to your PRIME 💪

A lot of men spend the second half of their lives telling stories about what their body used to be capable of.

I’d rather build a body that gives you new stories to tell.

New strength PRs at 40.
Getting leaner than you were at 30.
Running without thinking about your knee.
Sprinting again.
Playing sports with your kids and actually keeping up.
Looking in the mirror and realizing you’re in better shape than you were 10 years ago.

Getting older should change how intelligently you train.

It shouldn’t automatically lower the standard for what your body is capable of.

You don’t need to recreate the 25-year-old version of yourself.

Build a better one.

That’s the entire philosophy behind PRIME:

Your prime isn’t an age. It’s a standard.

I created the PRIME Pain → Performance Playbook for men 30+ who know they’re capable of more than the physical standard they’re currently living at.

A lot of runners aren’t being held back by their injury.

They’re being held back by outdated advice about what they’re supposed to do when something hurts.

“Stop running until it’s completely gone.”

“Just rest for a few weeks.”

“Don’t lift. You’ll make it worse.”

Then 3–4 weeks later you try running again… and somehow the exact same knee, hip, Achilles or back pain comes right back.

Because rest may calm symptoms down, but it doesn’t automatically build a body that can tolerate running again.

Good rehab should help you keep doing as much as you safely can while progressively rebuilding your capacity to run, lift, sprint and train.

Sometimes that means modifying mileage.

Sometimes that means temporarily changing intensity.

Sometimes that means getting stronger.

But the goal shouldn’t be to make your life smaller until nothing hurts.

The goal is to build your body back up until it can handle the life and training you actually want to do.

Runners: save this one for the next time someone tells you to “just rest.” 🏃‍♂️

Your schedule is busy.
Your goals are big.
Your rehab shouldn’t require you to rearrange your entire life around appointments to actually work.

These are CEOs, physicians, engineers, bankers, and competitive athletes who needed to get back to running, lifting, sports, work, travel and LIFE and they did it with online rehab.

Because virtual PT doesn’t mean less hands-on.

It means your rehab actually gets to live where your life happens.

We can see how you move, analyze your lifts and running, adjust your program as you progress, troubleshoot flare-ups when they actually happen, and build a plan around the things you need your body to be capable of doing.

You don’t need to be in the same room as your PT to get great rehab.

You need the right plan, the right progression, and someone paying attention to the details.

And these texts are exactly why I’ll die on that hill 👏🏼

You aren’t “broken.”

You’re just under-prepared for the life you want to lead 📉😤

It’s a story I see every single week: A high-level executive or a former athlete who spent their 20s moving with total freedom. You played basketball three times a week, you skied every winter, and you never thought twice about your knees or your back.

Then life happened 💼👨‍👩‍👧‍👦

The career took off. The kids arrived. The travel increased.

Gradually, the training sessions were replaced by meetings and the recovery time was replaced by emails.

You didn’t “get old”, you just transitioned into a lifestyle that is physically sedentary but mentally high-stakes.

The problem arises when you try to call on that “old athlete” for a weekend game or a ski trip. When the demand of the activity far exceeds your current capacity, your body sends a pain signal. Not because it’s damaged, but because it senses danger and its trying to protect you.

Stop shrinking your life to fit your pain 🛑

At athELITE, we don’t just treat the “soreness.” We build the foundation back up so that your body can actually handle the life you’ve worked so hard to build.

The biggest lie in rehab? “I need someone to fix me.”

Most people treat physical therapy like a car repair shop. They drop their body off for 60 minutes, expect a “tune-up” from a therapist’s hands, and then go back to their life.

But here is the hard truth: You are not a car. 🛠️🛑

You are a biological system that adapts to stress, not a machine that needs a mechanic. While manual therapy (massage, adjustments, scraping) is a great side dish, it is NOT the main course. It can open a temporary “window” of relief, but it cannot build the strength, stability, or tissue capacity you need to return to the court, field or the boardroom.

If your rehab only happens during the 60 minutes you’re in a clinic, you are ignoring the 10,000+ other minutes in your week. That is where the real results happen ⏳📈

Your body doesn’t care if you’re in a fancy clinic or your home gym, it only responds to progressive overload.

The goal of a high-level performance program isn’t to keep you dependent on a therapist for a weekly “fix.” It’s to give you the blueprint, the progression, and the autonomy to become your own best advocate.

Stop settling for temporary relief. Start building permanent capacity. 🤝

Swipe through to see why the “hands-on” model is failing you. 📲

From chronic shin pain to 7 marathons in 7 days on 7 continents. 🏃‍♂️🌍

A runner came to us dealing with constant shin pain. Every time he tried to bump up his weekly mileage, his shins would flare up and force him to stop. He was frustrated, stuck, and didn’t know how to move forward.

When we evaluated him, we realized something important: He wasn’t weak because he was injured. He was injured because he was weak; he wasn’t strong enough for the volume he was doing.

Plus, he was ramping up his mileage way too fast. His body just wasn’t ready.

Here’s what we did:

Step 1: We actually dialed back his running volume. I know, counterintuitive. But we needed to let things calm down.

Step 2: We shifted the focus to the weight room. Heavy lower body work, especially single leg work: squats, deadlifts, calf raises. We built a solid foundation.

Step 3: Once that baseline strength was there, we slowly and progressively layered his running volume back in. No sudden spikes.

The result? His body could finally handle the high-intensity, high-volume work he wanted to do. He went on to complete the Great World Race: seven marathons, seven days, seven continents 🔥

The lesson: If your mileage is going up and things are starting to hurt, don’t just push through it.

Let’s look at your strength, your capacity and your programming.
