A use-of-force report tells the story from one important perspective. But in correctional litigation, another question often matters:
Did the force occur within the larger framework of how the facility was supposed to manage that situation?
That means understanding what led up to the encounter, how staff responded as circumstances changed, what correctional policies and training required, and whether the response was consistent with the situation staff faced at the time.

Depending on the circumstances, leadership may need to look at applicable policies, staff responsibilities, supervision, training, communication, documentation, classification or housing decisions, staffing, incident response, and whether daily practices were consistent with written procedures.
The objective should be to understand the system, not simply find someone to blame.

Being short-staffed does not automatically tell you whether a correctional facility failed in a particular situation.
The more useful questions are specific.
Who was assigned to the relevant post? Was that post covered? What responsibilities did staff have at the time? Were required rounds or supervision performed? Did overtime, vacancies, or reassignment affect a correctional function connected to the incident?
In our correctional expert witness work, staffing records may need to be evaluated alongside post orders, housing logs, video, policies, incident records, and the actual timeline.
Staffing can also overlap with supervision, classification, emergency response, security, healthcare coordination, and other jail or prison operations.

An audit is important, but PREA compliance also has to function in the day-to-day correctional environment.
Policies may look appropriate on paper while operational gaps develop in areas such as staff training, supervision, reporting, investigations, inmate management, documentation, or communication. When an inmate dies by su***de in a jail, the final incident report may document what happened at the end. It may not explain everything that occurred beforehand.
In our correctional wrongful death and jail su***de reviews, we may need to look across multiple parts of the institutional record. Depending on the circumstances, that can include booking and intake information, su***de-risk screening, mental health referrals, classification and housing records, observation records, officer rounds, staffing assignments, post orders, policies, training, video, healthcare records, and emergency-response documentation.
The larger question is often how information moved through the facility. If a risk was identified, where was it documented? Who needed to know? What action followed?
