How to image pacemaker leads? 🐗🔍

As you can see in these clips, imaging and assessing pacemaker lead position on echocardiography isn't easy. You need to find the right angles to locate the lead and confirm it's correctly placed.

Take a look at this pediatric case: An 11-year-old male patient presents with a sore throat and abdominal pain. Findings from abdominal palpation prompted an ultrasound examination. What is the most likely finding in this ultrasound loop and the most likely diagnosis?

A) The spleen is smaller than the adjacent kidney, suggesting splenic atrophy due to chronic disease
B) The spleen is enlarged compared with the adjacent left kidney, suggesting splenomegaly in infectious mononucleosis
C) The left kidney is enlarged, suggesting acute pyelonephritis
D) Free fluid is seen around the spleen, suggesting splenic rupture

Note: The size of the spleen can vary significantly; therefore, it is best evaluated in relation to the adjacent kidney. The spleen should be roughly comparable in size to the kidney. 💡
