A simple system can make managing dementia care feel a little more organised. 💙

One practical strategy I recommend to families is creating a home medical file. A simple ring binder can keep medical letters, appointment details, test results, important information and questions together in one place.

Take it with you to appointments and add new information as it comes up.

It can make communication with health professionals easier and, importantly, reduce some of the mental load of trying to remember everything yourself.

Save this Reel for later or share it with a family member or carer who might find it useful.

Hi everyone, I’m a neuro physio and dementia consultant who is passionate about supporting people living with dementia and their care teams.

I’m offering a small number of dementia consultation visits for people and teams who would like support with:

• Understanding complex dementia-related care needs and changes in behaviour
• Finding ways to connect when communication or engagement is difficult
• Meaningful activities and cognitive stimulation suited to the person’s strengths and interests
• Mobility, transfers, positioning and comfort
• Practical guidance for support staff to feel more confident in their approach.

My role isn’t to replace your existing physio, OT or PBS practitioner. I work alongside them, families and support staff, bringing a dementia and neurorehabilitation perspective to assess the person’s needs, help everyone understand what may be happening and adapt their approach to everyday care.

📍 In-person visits within 20 km of 6156, with online support Australia-wide.

Eligible services may be billed under the NDIS or Support at Home physiotherapy item code, depending on the person’s plan and support provided.

Please message me if you’d like to discuss someone you support.

A dementia diagnosis can bring answers, but it can also leave you with so many new questions.

What is happening?
What might help?
Who can support us?
Where do we go from here?

People often wait a long time to receive a diagnosis, only to be left without clear guidance about what they can positively do next.

You don’t need to understand everything straight away.

The right information, explained simply, can help you make sense of what may be changing, support everyday life and relationships, and discover the therapy and support options that can help.

Understanding Dementia™ provides practical, therapist-guided education to help you feel clearer and more confident moving forward.

A dementia diagnosis doesn’t mean we stop looking at what is possible. 💙

There can be a lot of uncertainty about what comes next. But there are still meaningful ways to support someone living with dementia.

We can build on their strengths, support confidence and connection, and find ways to make everyday life work a little better.

Sometimes, it starts with changing the questions we ask.

Instead of only asking, “What is becoming harder?”, we can also ask:

✨ What is going well?

✨ Could I understand this better?

✨ What can I do to help?

Every person living with dementia is different, so the right support will look different too.

Families don’t always need more information. They need information that helps them understand what they can do, what may help and what support is available.

That’s why, at Dementia ConnectEd®, understanding comes first.

If you’d like to better understand dementia and explore practical ways to support someone you care about, visit the link in my bio or email [email protected].
