No technology company sat down and decided to hand decisions over to AI. It happened one system at a time.
Something started triaging claims. Something else started setting prices. Then contracts. Each step made sense on its own. Together they changed how decisions are actually being made inside the business, and how fast.
The uncomfortable part isn't that AI gets things wrong. It's that it can make the same wrong call a thousand times before a human sees the first one. And every one of them is still your decision.

Four questions worth putting on a board agenda this quarter:

Major failures rarely start with major decisions.

On 5 January 2024, Alaska Airlines Flight 1282 became a global headline minutes after take‑off. A door plug panel - designed to remain sealed for the life of the aircraft - blew out mid‑air, tearing a hole in a Boeing 737 MAX 9.
No lives were lost.
But confidence was.
The investigation didn’t uncover a technical mystery.
It uncovered something more uncomfortable.
The panel had been improperly installed.
Controls existed. Processes existed.
