The patient, a 49-year-old male, was at high risk of limb amputation following diagnosis and management of necrotizing fasciitis. Having undergone angioplasty, a reverse sural artery fasciocutaneous flap reconstruction was performed. The critical question was whether the flap would survive.

SnapshotNIR gave Dr. Mishko objective StO2 readings at specific intervention checkpoints at the point of care to help guide critical clinical decisions.

Flap failure is a feared complication in limb preservation surgery and one of the hardest outcomes to predict using clinical exams alone, especially in patients with compromised vascular status.

⚠️ Blood flow doesn’t always tell the whole story ⚠️

A patient can have adequate blood flow through the larger vessels, yet still have compromised oxygen delivery at the tissue level.
Why? Because macrovascular assessment doesn’t necessarily identify dysfunction in the small vessels responsible for delivering oxygen where it’s needed most.

The distinction matters: Small vessel arterial disease can impact tissue viability and a wound’s ability to heal, even when larger vessels appear adequate.

The next day this was confirmed with follow up imaging, giving the clinical team confidence in their intervention.
The result was a wound prepped for a split thickness skin graft in the upcoming weeks, and ultimately, a limb that didn't have to be amputated.
