ConnectMyHealth is a free, secure, portal where you can access many of your hospital health records from participating hospitals across southwestern Ontario, including Hamilton Health Sciences. It's especially helpful for those who receive care at multiple hospitals, providing a convenient, consolidated view of your records in one place.

The TAB Singers in Hamilton are a community vocal group for individuals with chronic lung conditions, organized through Compass Community Health. If you are interested in learning more, please call 905-523-6611 and ask for our Chronic Disease Management programs.

Over 600 community members came out this week to enjoy BBQ, live entertainment, and summer fun at our yearly Park N Party. We can’t wait to do it again next year!
