Radiation Overuse in Intensive Care Units 11/09/2026

🔝 This Friday we are bringing to you another Editor’s recommendation from our scientific editors — review titled "Radiation Overuse in Intensive Care Units"

☢️ "Radiological imaging is essential for acute patient management in Intensive Care Units (ICUs); however, it introduces the risk of exposure to ionizing radiation. This review synthesizes research on radiation exposure in ICU settings, highlighting its rise during the COVID-19 pandemic and the rise in repetitive imaging. Our analysis extends to radiation safety thresholds, revealing that some ICU patients exceed the diagnostic reference level, emphasizing the need to balance diagnostic utility against potential long-term risks, such as cancer. Prospective studies have demonstrated an increase in the median cumulative effective dose in patients with trauma over time, prompting calls for improved awareness and monitoring. Moreover, innovative dose-reduction strategies and optimized protocols, especially in neuro-ICUs, promise to mitigate these risks. This review highlights the essential but risky role of radiological imaging in critical care. It advocates for rigorous radiation management protocols to safeguard patient health while ensuring the continuity of high-quality medical care."

Article Title: Comparative Sensitivity of MRI Indices for Myelin Assessment in Spinal Cord Regions

🔹Although multiple magnetic resonance imaging (MRI) indices are known to be sensitive to the noninvasive assessment of myelin integrity, their relative sensitivities have not been directly compared. This study aimed to identify the most sensitive MRI index for characterizing myelin composition in the spinal cord’s gray matter (GM) and white matter (WM).
▫️MRI was performed on a deer’s ex vivo cervical spinal cord. Quantitative indices known to be sensitive to myelin, including the myelin water fraction (MWF), magnetization transfer ratio (MTR), the signal ratio between T1- and T2-weighted images (T1W/T2W), fractional anisotropy (FA), mean diffusivity (MD), electrical conductivity (σ), and T1, T2, and T1ρ relaxation times were calculated. Their mean values were compared using repeated measures analysis of variance (ANOVA) and post hoc Bonferroni tests or Friedman and post hoc Wilcoxon tests to identify differences across GM and WM columns possessing distinct myelin distributions, as revealed by histological analysis. Relationships among the indices were examined using Spearman’s rank-order correlation analysis. Corrected p < 0.05 was considered statistically significant.
🔹All indices except σ differed significantly between GM and all WM columns. Two of the three WM columns had significantly different MWF, FA, MD, and T2, whereas one WM column had significantly different MTR, σ, T1, and T1ρ from the others. A significant moderate to very strong correlation was observed among most indices.
Conclusions: The sensitivity of MRI indices in distinguishing spinal cord regions varied. A strategic combination of two or more indices may allow the accurate differentiation of spinal cord regions.

🦠 Title: A Non-Invasive, Label-Free Method for Examining Tardigrade Anatomy Using Holotomography

🔹Holotomography is an advanced imaging technique that enables high-resolution, three-dimensional visualization of microscopic specimens without the need for fixation or staining. Here we aim to apply holotomography technology to image live Hypsibius exemplaris in their native state, avoiding invasive sample preparation procedures and phototoxic effects associated with other imaging modalities.
▫️We use a low concentration of 7% ethanol for tardigrade sedation and sample preparation. Holotomographic images were obtained and reconstructed using the Tomocube HT-X1 system, enabling high-resolution visualization of tardigrade anatomical structures.
🔹We captured detailed, label-free holotomography images of both external and internal structures of tardigrade, including the digestive tract, brain, o***y, claws, salivary glands, and musculature.
▫️Our findings highlight holotomography as a complementary high-resolution imaging modality that effectively addresses the challenges faced with traditional imaging techniques in tardigrade research.

📲 We invite you to read our cited review "Calcified Nodules:
A Diagnostic Challenge in Clinical Daily Practice"!

🔎🫁 "Calcified lung nodules are frequently encountered on chest imaging, often as incidental findings. While calcifications are typically associated with benign conditions, they do not inherently exclude malignancy, making accurate differentiation essential. The primary diagnostic challenge lies in distinguishing benign from malignant nodules based solely on imaging features. Various calcification patterns, including diffuse, popcorn, lamellated and eccentric, provide important diagnostic clues, though overlap among different conditions may persist. A comprehensive diagnostic approach integrates clinical history with multimodal imaging, including magnetic resonance and nuclear medicine, when necessary, to improve accuracy. When imaging findings remain inconclusive, tissue sampling through biopsy may be required for definitive characterization. This review provides an overview of the imaging features of calcified lung nodules, emphasizing key diagnostic challenges and their clinical implications."

📲 Title: Musculoskeletal Pitfalls on Molecular Imaging Studies of Oncologic Patients: How to Stay Out of Trouble

📑 "An increasing amount of molecular imaging studies are ordered each year for an oncologic population that continues to expand and increase in age. The importance of these studies in dictating further care for oncologic patients underscores the necessity of differentiating benign from malignant findings, particularly for a population in whom incidental findings are common. The aim of this review is to provide pictorial examples of benign musculoskeletal pathologies which may be found on molecular imaging and which may be mistaken for malignant processes. Imaging examples are provided in the form of radiographs, bone scintigraphy, computed tomography, and fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) scans. Special attention is paid to specific features that help narrow the differential diagnosis and distinguish benign from malignant processes, with the goal of avoiding unnecessary invasive procedures."
