A short-term delayed implant placement protocol is used in cases like this, with a long-standing purulent abscess, because the cancellous bone can be infiltrated by infection

If infection develops after immediate implant placement, this may create an iatrogenic factor and make future implant placement more difficult

The tooth is extracted and the socket is carefully cleaned. The socket is then temporarily covered with collagen for several days. In this case, the healing period was 4 days

This short healing period allows the body’s natural immune response to control the infection and allows us to confirm that the socket is healing without complications.

After 4 days, the granulation tissue and remaining collagen are removed from the part of the socket planned for implant placement and bone grafting. The implant is then placed according to the immediate implant placement protocol, and the peri-implant gap is filled with a bone graft

The socket is then sealed with a customized healing abutment or an immediate provisional crown

Biotype Conversion | BOPT Approach

This case shows how provisional restorations can shape the soft tissues after vertical preparation.

The key is to determine the gingival level through the provisional contour, establish a landing zone for tissue support, and create horizontal space for the gingiva to thicken.

One month later, we can see a fuller gingival contour and developing papillae. This is biotype conversion in action.

And a question I often hear: Can we use a rubber dam for adhesive cementation in a BOPT case? Yes. We show the isolation and final cementation in this case too.

The starting point: deeply prepared teeth, old crowns and compromised soft tissue contours

After removing the crowns, we rebuilt the foundations, performed vertical preparations and used provisional restorations to shape the emergence profile

Over the following weeks, the tissue thickened and matured, with better gingival contours and papilla development

The final restorations were adhesively cemented under rubber dam isolation. Yes, isolation is possible in a BOPT case - even with deep preparations

🧠 Gingiva Never Lies.

Deep preparation carries a risk of gingival injury and related complications. This is especially relevant in retreatment, when we inherit existing preparation margins.

Following a clear protocol helps reduce these risks and achieve more predictable outcomes.

First, we need to understand biologic width - or supracrestal tissue attachment. The restoration margin must be assessed in relation to these structures. Careful bur handling alone cannot compensate for encroaching on the attachment.

Then comes how we prepare close to the gingiva.

When working deep within the sulcus, where contact with soft tissues is possible, our protocol includes:

• An electric motor with a speed-increasing contra-angle
• Fine-grit, red-band burs
• A reduced bur speed of 50,000–100,000 rpm
• Light, controlled pressure and adequate cooling

These measures help minimise trauma from incidental gingival contact, although they cannot eliminate the risk entirely.

At this stage, we avoid air turbines and coarse-grit burs. Instrument control and gentle handling matter.

The next step is well-fitting provisional restorations with precise margins and carefully designed contours. They support tissue healing and help shape the soft-tissue profile.

🧠 A composite veneer on a single central incisor is a completely different level of challenge.

In the previous posts, we showed smaller composite restorations - localized corrections where subtle changes were enough to improve the smile.

This time, the task was different: a full composite veneer on a single central incisor, with one main goal - to make the restoration disappear next to the natural tooth.

And this is about much more than simply choosing the right shade.

You need to reproduce value, translucency, opacity, internal optics, shape, macro- and microtexture, and the way the surface interacts with light. A small discrepancy in just one of these parameters can make the two central incisors look completely different.

That’s why restoring a single central incisor is such a great test of your understanding of natural tooth optics and composite materials.

In the original post, we show the complete clinical sequence: optical planning → wax-up → layering → replication of the adjacent tooth texture → final result under different lighting conditions.

🧠 Long-term functional temps: which cement do we choose?

Functional provisionals allow us to test occlusion, aesthetics, and patient comfort - sometimes over several months. Stable fixation is essential throughout this process.

Conventional temporary cements, such as Temp-Bond, are convenient and allow relatively easy removal. But longer wear places different demands on cementation.

In our practice, we have encountered loss of retention, biting sensitivity, and unpleasant odour when removing provisionals. Sometimes patients notice the odour themselves.

These issues are not determined by cement alone. Marginal fit, occlusion, tooth condition, and hygiene all matter. But cement selection remains an important part of the protocol.

For long-term functional provisionals, our choice is polycarboxylate cement, such as Durelon.

Yes, old-school. But we value its rigidity, interaction with tooth structure, and reliable retention. Our goal is a stable restoration, a good marginal seal, and patient comfort throughout the functional test drive.

Stronger retention also means planning ahead for removal.

What about non-retentive provisionals, such as overlays and veneer-onlays?

In our original Facebook post, we demonstrate our approach: polycarboxylate cement with additional adhesive reinforcement using flowable composite on the palatal or lingual side.
