5 NSTEMI patients. One striking pattern.

Managed 5 patients between 40–50 years of age, all presenting with NSTEMI.

The common finding?

- Critical LAD disease with 90–99% stenosis.
- Patients had preserved LV function, highlighting that severe coronary disease can exist despite preserved ventricular function.

One patient with in-stent restenosis (ISR).

Instead of placing another stent patient managed with:
✔️ Lesion preparation with a cutting balloon
✔️ Followed by a drug-eluting balloon (DEB/DCB)

The final angiographic result was excellent, avoiding an additional metal layer while achieving optimal vessel patency.

These cases are a reminder that:

* Age is no longer a reliable predictor of severe coronary artery disease.
* NSTEMI can conceal critical coronary lesions despite preserved LV function.
* Contemporary ISR management with meticulous lesion preparation and DEB technology offers an effective alternative in selected patients.
