The Silent Crisis of Unemployment Among Healthcare Workers Especially Nurses and Midwives
Dear Stakeholders,
There is a high level of unemployment among healthcare workers at all levels and cadres.
This is a crisis that deserves urgent attention. Many of our continuing professional development (CPD) participants have told us they wish to enrol in our programs but cannot, because they have no jobs to support themselves or pay for training.
They have papers but no one is willing to employ them. They cannot find jobs. Many are frustrated, stressed and some even regret joining the health professions.
My writing today focuses solely on the high rates of unemployment among Nurses and Midwives sharing the gaps and proposing what we can do about them as a country.
We are producing health workers for a labour market that is not absorbing them.
Government and private sector employers are already stretched and cannot employ everyone. At the same time, many health workers are not prepared or supported to create their own opportunities.
Where are the gaps?
🌼The system trains job seekers, not job creators. We are taught to study hard and then look for white collar jobs.
Our curricula still lack alignment with local community problems that healthcare workers could help solve using their knowledge, skills and resources creating opportunities for themselves while serving people.
🌼Government and the private sector are too full to employ the unemployed. They cannot absorb the growing number of health workers.
🌼Start-up barriers are too high. It takes a Registered Nurse more than 10 years to open a private nursing or domiciliary homes.
The long waiting period makes it very difficult for Nurses to start their own practices.
🌼Attitude and mindset remain a challenge. Many healthcare workers still expect formal employment rather than exploring self-employment and innovation.
🌼Exploitation is widespread. In the private sector, nurses face unregulated salary scales, manipulation and overwork. In many government facilities, overwork is also common.
🌼Progression bottlenecks exist. The two-year waiting or experience requirement between certificate and diploma levels is a gap. This should be reviewed so that people can upgrade and pursue areas of specialisation without unnecessary delay.
We recommend that the following issues be addressed,
1.Review curriculum gaps to include community problem-solving, entrepreneurship, leadership and practical local solutions.
2.Introduce intensive empowerment programs for nurses and midwives.
3.Promote attitude and mindset change,from job seeker to job creator.
4.Design tailored entrepreneurial programs that address local challenges with local solutions.
5.Lift or shorten the long 10-year restriction for Nurses to open their own clinics, while maintaining quality and patient safety standards.
6.Regulate private-sector salaries and working conditions to end exploitation.
7.Create clear progression pathways by removing unnecessary waiting periods and expanding specialisation opportunities.
8.Build partnerships for labour-market opportunities, including in the diaspora, where nurses can do decent work for which they trained and be paid fairly.
9.Strengthen collaboration among government, regulators, training institutions, employers, professional associations, development partners, and health workers themselves.
Unemployment among nurses and midwives is not just an employment issue it is a health system and national development crisis.
We cannot keep producing graduates for jobs that do not exist. We must build an ecosystem that supports Nurses and Midwives as both employees and job creators.

1. UNMC – Uganda Nurses and Midwives Council
The Regulator (The Gatekeeper of Quality)
