As we enter into the second curve of value based healthcare delivery the triple aim approach serves as a key measurement indicator. A primary goal of the triple aim approach is to provide patient-centered care designed around wellness and prevention that actively engage individuals in their own health. The collaboration between faith communities and healthcare systems can play a pivotal role in the changing dynamics of healthcare. CORE Institute can assist in establishing faith-based programmatic frameworks such as faith community nursing and congregational health promoters to provide an extension beyond the traditional healthcare setting. Community Outreach Resources and Education (CORE) Institute is an independent center of learning and community development with a specific focus on connections to faith-based organizations.
