Start at least 90 days before your IVF cycle. I've been saying it for years, and I'm not changing my answer.
But for some of you, 90 days is your starting line, not your finish line. That's you if you have PCOS, a history of endometriosis surgeries, or more than one failed IVF. It's also you if you're not sleeping, running on high stress, low on iron, or your cycles have gotten shorter.
Here's why 90 days matters: an egg takes about three months to mature. That's the window where we support the egg you'll actually use.
And here's why some of you need more: clearing out what your body has too much of is faster than building up what it's missing. When you're depleted, rebuilding takes time. You can't rush it in the last few weeks before a transfer.
My approach is simple: prepare, don't react. I'd rather meet you months before your next transfer than the week before.

If your nose runs every October like clockwork, the next few weeks are your window to get ahead of it.
Fall allergy season in Wisconsin comes in waves. Ragweed hangs on until the first hard frost. Then the leaves come down, sit wet in the yard, and grow mold. Raking sends it straight into your face. Then the furnace kicks on and dries out every room in the house. By the end of October, plenty of people are waking up stuffed, rubbing itchy eyes, and going through a box of tissues a week.
Chinese Medicine has a useful way to look at this. Fall is Lung season, and in that system the Lungs handle more than breathing. They're in charge of your nose, your skin, and the barrier that decides what gets in and what your body overreacts to. When that barrier dries out and gets irritated, your body overreacts to things it used to handle fine.
In plain English: fall allergies are partly about what's in the air, and partly about how ready your defenses are when it shows up. That second part is something you can work on now.
Five things I tell patients every fall:
Moisten from the inside. Have warm drinks, soup, and cooked pears. Go easy on ice-cold drinks this season.
Rinse after you rake. Do a saline rinse made with distilled or boiled water, then shower after yard work so the leaf mold doesn't come to bed with you.
Humidify the bedroom. Aim for 30 to 50 percent humidity once the furnace is on. Going above 50 feeds mold, so don't overdo it.
Cover the back of your neck - I tell everyone this! Wear the scarf on windy days.
Protect your sleep. Your body does its repair work overnight, and a barrier that never gets a break stays reactive.

The last appointment of my day never reschedules.
This is Roscoe. He's my shepherd mix rescue and he recently just turned eight.
He doesn't care how many patients I saw or how long the drive home was.
On my days off, he knows exactly where we're going as soon as I put my shoes on.
Pup cup time! Off to 7 Brew we go.
In the evening before dinner, it's the dog park.
He's not there to play with the other dogs. He's there to find every spot they p*ed on and p*e over it.
Every single one. Then we come home and he enjoys his Ollie like he just finished a hard day's work.
I used to think of this stuff as extra, something to fit in if I had time. Now I know it's part of taking care of myself. Watching him make his rounds at the park, or down a pup cup, brings me absolute joy.
I spend all day telling people the body heals best when the pressure comes off. Roscoe makes sure I take my own advice.

Waking up soaked at 2 AM is not random, and it is not just "getting older."
In your 40's, your internal thermostat gets touchy. The comfort zone it used to hold easily narrows to a sliver, so a shift that your body once ignored now reads as an emergency, and it slams the cool-down button. Heat surge. Sweat. Wide awake with your heart pounding.
Nighttime stacks the deck. If dinner was light on protein or the w**e was close to bedtime, blood sugar dips in the small hours. Your body releases stress hormones to correct it, and that surge is often the thing that flips the switch.
What helps steady the system: real protein at dinner, more space between w**e and bed, a genuinely cool room, and a nervous system that is not running on alert all day. A body that spends the day under pressure guards harder at night.

If your knees, hips, or shoulders started aching in your 40s and you can't point to an injury, you are not suddenly old. Something changed on the inside.
Estrogen does quiet work all over the body. It helps keep inflammation calm and supports the tissue that cushions and lubricates your joints. In the years before menopause it stops being steady and starts to swing. For a lot of women, the first place they feel that is stiffness. Stiff hands in the morning. Achy knees on the stairs. A shoulder that complains for no reason it can name.
So they stretch more. Blame their workouts. Or quietly start moving less, which makes everything worse. But the joints were never the problem. The terrain around them changed.

For ten years, her back "went out" about twice a year. She could feel it coming. A wrong twist, a heavy laundry basket, one bad sneeze. Then a week on the couch, walking like a question mark, canceling everything on the calendar.
She'd been told her back was weak. So she did the stretches. Bought the fancy mattress. Braced herself every time she picked something up. The episodes kept their schedule anyway.
Here's what we found: her back wasn't fragile. It was guarded. After that first bad episode years ago, her body learned to treat the area like a danger zone. A nervous system standing guard will clamp those muscles down hard at the smallest wrong signal. That is what "going out" actually is. Not a spine failing. An alarm firing.

Cold hands and feet all day. Then at 2 AM you’re throwing the covers off. Those sound like opposite problems. They’re the same one.
Heat is stuck in the center and isn’t reaching the edges. During the day your hands and feet don’t get their share. At night everything slows, that heat has nowhere to go, and it surfaces. You wake up hot in a 66 degree house.
The mistake is treating them separately. Something cooling for the night, something warming for the day, and they cancel each other out. That’s why nothing has worked.

Let's stop being polite about this.
You are sitting in a meeting, in a car, at your kid's game, feeling the heat climb up your chest like someone opened an oven behind you.
You've got a cardigan you take off and put on nine times a day. You changed the sheets at 2 AM again. You are functioning on chopped-up sleep and everyone around you is getting the version of you that got four hours.
And you're angry. Not delicate. Not "a little irritable." Angry, in a way that surprises you, at people who did not earn it.
Then you go ask for help and get told this is just your season now. Ride it out. It'll pass in a few years.
A few years. That's a thousand nights. Nobody would tell a man to ride out a thousand bad nights.
Here's what's actually happening. Your internal thermostat has been calibrated your entire adult life by hormones that are now swinging. When it loses its reference point, the range it calls normal gets razor thin, so a tiny bump in your core temperature sets off a full emergency response. Blood vessels dump heat to your skin, you flush, you soak, and twenty minutes later you're freezing.
In Chinese medicine we've had language for this for centuries, and it's more useful than it sounds. Picture a candle. There's wax and there's flame. When the wax runs low, the flame doesn't shrink. It burns hotter, flares, and throws heat everywhere. That's your flush. That's the 2 AM soak. That's why you're burning up and running on empty at the same time, which never made sense to anyone who told you to just ride it out.
The rage is part of the same picture. When the cooling side of your system is depleted, everything runs hot, including your fuse. Add a thousand broken nights on top of it and of course you're snapping. That's a body under load, not a personality.
We don't smother the flame. We rebuild the wax. Nourishment, sleep, circulation, and taking real weight off the system with acupuncture, herbal medicine, and specific changes to how you eat and move.
And understand what the broken sleep is costing you beyond the heat. Your energy, your patience, your memory, and your body holding weight it never held before. Every one of those gets written off as age. Most of them start at 2 AM.

Migraines are the condition I have the most success with.
Here's why, and it is not a secret technique.
Because "migraine" is not one thing. It is a symptom with several completely different drivers, and the treatment that works depends entirely on which one is yours.
Some women's migraines track their cycle. They show up in the same window every month, which tells me the driver is hormonal, and that is what I treat.
Some come from the neck and shoulders. Old whiplash, a desk setup, jaw clenching, structural tension that starves the head of good circulation. Different problem, different plan.
Some are digestive. Sluggish digestion, blood sugar swings, a food you eat constantly and never suspected. When we sort that out, the head follows.
And plenty of people have two of these running at once, which is exactly why the single thing they tried never worked.
This is why I ask so many questions that seem to have nothing to do with your head. Your cycle, your sleep, your digestion, your jaw, what you ate today.
