Women keep finding one another.

Over the past few days, I’ve had several unexpected conversations about midlife health.

One woman shared that she was still experiencing hot flashes years after her final period. She wanted to know whether hormone therapy could still be an option for her—not an automatic yes or no, but a question worthy of an individualized conversation about potential benefits and risks.

Later, while shopping for clothing that would feel cooler and more comfortable, a sales associate began talking openly about her own symptoms and what she had been trying.

Another woman told me she has been reading, saving, and returning to the information I share.

What stayed with me was not simply that these women had questions.

It was the relief that appeared when they realized they did not have to explain why the questions mattered.

Maybe you know that feeling.

You know something has changed, but you are still trying to find the right words for it.

You are not asking for menopause to be the automatic answer.

You are asking for someone to listen, consider the whole picture, and help you understand what may be happening.

Being heard is not a diagnosis.

But it is often where thoughtful care begins.

Women are already having these conversations—in stores, workplaces, homes, and community spaces. They are helping one another find language for changes that were once endured quietly.

Your hormone test came back “normal.”

Okay—but that does not get to end the conversation.

You’re awake at 3 a.m. Your period has changed. Your mood, energy, or body does not feel like your own. You know something is different.

Here is the part women are not always told: hormones during perimenopause do not hold still for the blood draw. Estrogen and FSH can fluctuate considerably, so one result may capture a higher day, a lower day, or somewhere in between.

That does not make testing useless. It means the number needs context.

For most women over 45 with a typical presentation, perimenopause is assessed through age, symptoms, menstrual history, medications, medical history, and other possible explanations—not one isolated hormone result.

You are not being dramatic. And you should not have to build a courtroom case to prove that something has changed.

A lab result is a snapshot.

Your health is the story.

You can still have a period every month and be in perimenopause.

That surprises a lot of women.

Perimenopause starts when your cycles begin to change — and early on, that change is easy to miss. A cycle that runs a few days shorter, then a few days longer. You’re still bleeding every month, so nothing looks obviously wrong.

And often what you notice first isn’t your cycle at all.

Sleep may change.
Hot flashes or night sweats may begin.
Mood may feel different.
Headaches may change.
You may notice brain fog, breast tenderness, or changes in how you feel before your period.

Eventually the cycle changes become harder to miss — longer gaps, skipped months. But you don’t have to wait until your periods are dramatically irregular — or gone — before bringing up symptoms that are affecting your life.

And not every symptom in your forties is perimenopause. Thyroid disease, anemia, sleep disorders, medications, mood conditions, and other medical issues can overlap with it.

That’s why naming the transition matters — and so does making sure we’re not blaming everything on it.
