A general contractor asks to be named on your policy. You send over the certificate. Everyone moves on.

A certificate is evidence of insurance on the day it was issued. It does not by itself add anyone to your policy. 𝗔𝗻 𝗮𝗱𝗱𝗶𝘁𝗶𝗼𝗻𝗮𝗹 𝗶𝗻𝘀𝘂𝗿𝗲𝗱 𝗲𝗻𝗱𝗼𝗿𝘀𝗲𝗺𝗲𝗻𝘁 is what does that. Two different documents, and only one of them extends your coverage to the other party.

The contract requires an endorsement and only a certificate gets sent. The endorsement names the wrong entity. The coverage is written for ongoing operations when the contract also requires completed operations. Nobody notices until there is a claim.
