If you filled out a Toys for Tots Form at Ability Health and Rehabilitation, LLC, please call the office at 208-333-9578. The forms were turned in to the Marines, but the actual forms have been lost. Be prepared to give your name, child s*x and age so we can match up your order for you. I apologize for this hassle. Merry Christmas.
