1️⃣ Community Health Worker – English Speaking
Seeking an energetic, community-minded individual who is familiar with Chicago’s West Side and comfortable engaging residents throughout African American and other diverse communities.
2️⃣ Bilingual Community Health Worker – English/Spanish
Seeking an individual who is fluent in English and Spanish and comfortable conducting outreach throughout Spanish-speaking, African American, and other diverse West Side communities.
💼 POSITION HIGHLIGHTS
💰 $22/hour
📅 Contract positions – grant-funded through December 2026
🕣 Monday–Friday | 8:30 a.m.–5:00 p.m.
🌙 Must be available for occasional evenings, Saturdays & Sundays
💵 Paid monthly – Independent contractor; taxes are not withheld
🚗 Reliable transportation required
📍 Familiarity with Chicago’s West Side strongly preferred
❤️ WHAT YOU’LL DO
• Conduct community outreach throughout nine Chicago West Side communities
• Provide free blood pressure screenings and hypertension education
• Connect residents with healthcare, social services, and community resources
• Participate in health fairs, neighborhood events, and wellness programs
• Assist with food and community-resource distribution
• Build positive, trusting relationships with residents and community partners

We need people who are outgoing, dependable, compassionate, energetic, and not afraid to approach and engage people in the community.
Experience in community outreach, healthcare, public health, social services, or customer service is preferred.

Qualifications
