Grant Jacobson had the opportunity to spend time in Dallas with our sister agency, Tech Insurance Agency, and the ITServe Alliance Dallas Chapter for a Workers’ Compensation Safety & Compliance Workshop—and it was well worth the drive.

The room was full of IT staffing company owners looking for practical strategies to reduce risk, control workers’ compensation costs, and better protect their teams.

One of the biggest takeaways: five simple controls that can have an outsized impact on claim outcomes—from matching the right employee to the right assignment to reporting injuries immediately instead of waiting days. Small habits. Real results.

Great conversations, a packed room, and a reminder that the best risk management happens long before a claim is ever filed.

Two law firms were sanctioned this year after filing a court brief with AI-generated legal citations that turned out to be fabricated. Nine of the twenty-seven citations were fake. When the judge flagged two suspicious ones, the firms filed a corrected version that still had six errors left in it.

The brief was struck from the record. The firms had to disclose the misconduct to their client and pay over thirty thousand dollars in fees. It's not an isolated case, either. Similar incidents have shown up in courts across the country as AI tools get folded into professional workflows faster than review processes can keep up.

Here's what makes this relevant beyond law firms: professional liability policies weren't written with AI-generated errors in mind. Many are silent on the issue, and insurers are starting to close that gap with new exclusions rather than broader coverage. If your business uses AI in any client-facing capacity, whether that's research, drafting, or advice, it's worth checking how your E&O policy actually treats those risks before you find out the hard way.

AI can genuinely make work faster. It still needs a human checking the output before it goes out the door.

CGL insurance covers a lot, which is exactly why the exclusions catch people off guard. A policy that seems comprehensive on the surface can still leave real gaps once you look at what it specifically won't pay for.

A few exclusions worth knowing:
✅ Contractual liability: you generally can't pass someone else's liability onto your CGL policy just because a contract says so
✅ Liquor liability: if you serve alcohol, you likely need a separate endorsement
✅ Faulty products or services: that's professional liability territory, not CGL
✅ Coverage B restrictions: media and internet-related businesses often can't secure coverage B (personal and advertising injury) at all under a standard policy

None of this means CGL isn't worth having. It's foundational coverage for most businesses. But foundational isn't the same as complete, and the businesses that get caught off guard are usually the ones who assumed their policy covered more than it did.

Most coverage gaps don't come from bad decisions. They come from assumptions nobody double-checked. A few we see often:

‼️ Equipment breakdown: standard property insurance excludes mechanical or electrical failure, it needs its own endorsement
‼️ Contingent business interruption: standard BI only responds to your own property damage, not a supplier's
‼️ Employment practices liability: not covered under general liability, most businesses need a standalone EPL policy
‼️ Hired and non-owned auto: gaps show up when employees use personal or rental vehicles for work and the business assumes its commercial auto policy already has it covered

None of these show up until there's a claim. That's the frustrating part. The fix is simple though: a coverage review with someone who knows what questions to ask before renewal, not after.

The cyberattacks that make headlines are usually ransomware or a big data breach. What rarely gets mentioned is how the attacker got in to begin with, and increasingly, the answer is a stolen password.

Infostealer malware sits quietly on an infected device, collecting login credentials and session tokens, then sends that information to an attacker who may not use it for weeks. That delay is what makes it dangerous. By the time a business notices anything wrong, the credentials have often already been sold and used.

Here's the part that catches a lot of companies off guard: infostealers can steal session tokens, not just passwords, which means they can sometimes bypass multi-factor authentication entirely. The attacker inherits an already-logged-in session and looks identical to a legitimate user.

MFA is still worth having. It just can't be the only layer.

Pair it with endpoint protection, least-privilege access, and a team that knows how to spot phishing attempts, and a stolen credential becomes a lot harder to turn into a real breach.

With crews out on job sites and properties seeing more outdoor foot traffic this time of year, a quick refresher on summer basics goes a long way.

☀️ Swimming or working near water: confirm conditions with local officials or lifeguards before anyone gets in
☀️ Insect bites and allergies: waterproof repellent and basic allergy medication on hand can keep a minor issue from turning into a bigger one
☀️ Hydration: don't wait until someone feels thirsty, build water breaks into the schedule
☀️ Grilling or outdoor cooking at company events: keep equipment at least 3 feet from anything flammable and never leave it unattended

None of this takes much effort, but it's the kind of small habit that keeps a good summer from turning into an incident report.
