Leucovorin went from headlines to a lot of confusion. 🧬 Folate needs a receptor to reach the brain, and some autistic kids have antibodies that may block it. Small trials showed improvements in language, especially in kids with those antibodies. But the largest trial was retracted in 2026, and the FDA approved leucovorin only for rare genetic cerebral folate deficiency, not autism.

šŸ„› On dairy: one small 2008 study found antibody levels dropped on a milk-free diet and rose again when milk was reintroduced.
Promising, but it measured antibodies, not symptoms. If you go dairy-free, replace calcium and vitamin D.

šŸ”„ On inflammation: these antibodies are an immune response, and research suggests inflammatory markers run modestly higher in autism on average. Sleep, gut comfort, whole foods, and treating infections are low-risk ways to support the immune system.

šŸ“š The research:
• 2013 (Molecular Psychiatry): In 93 autistic kids at a specialty clinic, about 75% of those tested had folate receptor antibodies
2018 (Molecular Psychiatry): A 12-week trial of 48 kids found leucovorin improved verbal communication vs placebo, most in kids with the antibodies.

• 2008 (Dev Med Child Neurol): In 33 kids with cerebral folate deficiency, antibody levels dropped on a milk-free diet and rose again when milk returned.

• 2026 (Molecular Psychiatry): A review of 98 studies found inflammatory markers modestly higher in autism on average, though the evidence was mostly weak.

• 2024 (Biomed Pharmacother): One study found lower antibody levels in autistic boys than in other boys, a reminder that results don’t all agree.

Promising for some, not proven. Talks to your provider, and check B12 first.

Every child is different, and that’s why I believe we have to look at the whole picture—not just one symptom.

The goal isn’t to blame one lab marker or assume we’ve found the answer.

It’s about gathering the right information, putting the pieces together, and using that information to better support the child in front of us.

Picky eating in autism isn’t a parenting failure. šŸ½ļø Research suggests autistic kids are about 5x more likely to have feeding challenges, often tied to sensory sensitivity. A narrow diet can mean less protein, calcium, and other nutrients the body uses for sleep, mood, and energy. No pressure, lots of safety, and bring it up with your provider or a feeding therapist. Education, not medical advice.

Your child’s gut is closely connected to their immune system, microbiome, digestion, and overall health.

šŸ”¬ This is one reason I like using testing strategically. The right test can give us additional information about things like bacterial balance, digestive function, gut immune defenses, and other patterns that may be easy to miss from symptoms alone.

But testing isn’t about finding one abnormal number and blaming it for everything.

It’s about putting the pieces together.

Sleep struggles in autism are common, and research suggests physiology plays a real role. šŸŒ™ Melatonin starts with the protein we digest. Serotonin is mostly made in the gut, and it’s about regulation, not just ā€œlowā€ levels. Iron only counts if it’s absorbed. Start with the body, build the routine, and work with your provider on supplements.

šŸ’™ When a child is struggling with constipation, I don’t want to only focus on getting them to have a bowel movement.

I want to understand the bigger picture.

How often are they going?

What does the stool look like?

Are they bloated?

Are they uncomfortable?

How is their diet?

And what might be happening within their gut environment?

🧩 The digestive system is complex, and there isn’t always one simple explanation for why a child is struggling.

That’s why I believe in taking the time to understand the individual child instead of automatically giving every child the same recommendations.

šŸ”¬ Sometimes testing can provide additional information that helps us ask better questions and make more individualized decisions about support.

Every child deserves an approach that considers their history, their symptoms, and their unique needs.

šŸ’™ Severe constipation doesn’t always mean a child simply needs more fiber.

This little girl was eating a good diet and getting plenty of fiber, but was still struggling with constipation severe enough to require an ER visit.

Her testing gave us additional clues about her gut environment, including yeast findings and short-chain fatty acids.

šŸ”¬ The goal isn’t to blame one marker. It’s to look at the whole picture—history, symptoms, diet, and testing—to guide a more individualized approach.
