She picked the plan with the $0 premium. Of course she did — she was healthy, it came with dental and a gym membership, and the math was obvious. Why pay $200 a month for something you don't need yet?

Then came the fall. The hospital stay. The doctor who said she needed two weeks of skilled rehab to walk safely again.

And the letter that said: denied.

I've watched this story play out more times than I can count, and the cruelest part is how predictable it is. Medicare Advantage plans are paid a flat amount per person every month — so every day of rehab they DON'T approve is money they keep. That's not a glitch. That's the business model. The gatekeeper between your doctor's orders and your actual recovery isn't a clinician. It's a cost center.

Here's what almost nobody tells you at signup: the denials usually aren't even defensible. When patients appeal skilled-nursing denials, the plans reverse themselves the overwhelming majority of the time. The care WAS justified. It just took a fight most exhausted, frightened families never know they're allowed to have — so the recovery quietly never happens.

And the door doesn't stay open. You can leave Advantage and go back to Traditional Medicare — but the supplemental coverage you'd want to bring with you can turn you down once you're already sick. The cheapest moment to choose is the exact moment the consequences feel furthest away.
