• Parent or legal guardian must be present during the piercing.
• Parent/legal guardian must present a valid government-issued photo ID.
• Minor must present a government-issued photo ID, passport, or school ID.
• A state-issued birth certificate (or certified copy) is required to verify the parent/legal guardian relationship.
• All documentation must be provided at the time of the appointment. No exceptions.
