After your Part B deductible, Medicare generally pays 80% of the approved amount for physician services and outpatient care. You owe the remaining 20%.

For $100,000 in approved services after the deductible, your share is $20,000.

Original Medicare doesn’t stop charging you coinsurance because you’ve already spent $20,000 that year.

And that’s just Part B.

Part A covers inpatient hospital care under different rules: a deductible per benefit period, plus daily charges for longer stays. It has no overall annual out-of-pocket ceiling either.

You also generally pay a monthly Part B premium. In 2026, that starts at $202.90.

People buy Medigap to help cover those expenses. Medicare Advantage offers an alternative with an annual limit for covered medical services. Medicaid or other supplemental coverage can also help eligible patients.

Those protections matter because Original Medicare alone leaves your exposure uncapped.

Before telling someone Medicare is free, explain how much of their retirement they can afford to spend on the remaining 20%.
