At Evergreen Post Acute, our in-house Speech-Language Pathology team helps residents with important areas of recovery, including safe swallowing, memory and problem-solving, attention, communication, and confidence in social interactions.

At Evergreen Post Acute, stroke rehabilitation combines skilled nursing care with individualized therapy to help residents work toward regaining strength, mobility, communication, cognitive skills, and independence.

Our in-house Physical Therapy, Occupational Therapy, and Speech-Language Pathology teams work closely with nursing to create personalized recovery plans, monitor progress, and adjust care as each resident’s needs change.

From rebuilding everyday skills to preparing for a safe transition home, our goal is to support residents through each stage of recovery with coordinated, compassionate care. 🌿

Our team can support residents who require TPN, tube feedings, complex wound care, medication management, daily nursing oversight, 24/7 skilled support and much more.

We also have our own in-house Physical Therapy, Occupational Therapy, and Speech-Language Pathology teams, allowing our therapists to work closely with nursing and the rest of the care team throughout each resident’s stay. Having therapy in-house helps create stronger communication, more coordinated care, and a more consistent approach to each resident’s recovery plan.

Whether someone is rebuilding strength after a hospitalization, working toward greater independence, managing complex medical needs, or preparing for a safe transition home, our goal is to provide the support they need every step of the way.

🌺🐐 We had some very special visitors at Evergreen Post Acute last week!

In celebration of Hawaiian Day, a few adorable friends from the farm stopped by to spend the afternoon with us! These little goats brought plenty of personality, curiosity, and smiles along with them. 💕

We’re excited to recognize Solomon Oli, CNA, for the incredible care and support he brings to Evergreen every day! Solomon is always willing to jump in and help wherever he is needed, consistently going above and beyond for both our residents and his teammates. He provides exceptional care, is dependable, and never hesitates to pick up where extra support is needed. His willingness to step up and be a team player makes a meaningful difference in our community, and we are so grateful to have Soloman as part of the Evergreen team! 💚👏
