While our name has changed, our dedicated team, our commitment to 24/7 care, and our deep roots in Central Oregon remain the same.

Offering hospice care, transitions, veterans support, bereavement, and now in-home primary care throughout Central Oregon. Hospice care is designed to focus on quality of life for the terminally ill patient. Our goal is to enable patients to be as comfortable and as free of symptoms and pain as possible, so that they live each day as fully as possible. We believe in treating the patient holistically, honoring their life and their wishes. Our philosophy is to provide support for the patient’s emotional, social, and spiritual needs as well as medical symptoms management as part of treating the whole person. We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

Today is the International Day of Older Persons, and it is a good moment to say something we believe all year. Growing older in the place you love, surrounded by the people and routines that make a life feel like yours, is no small thing. It is worth protecting.

For 47 years, our team has been walking alongside Central Oregon families in exactly those places, at kitchen tables and on front porches from Redmond to Prineville. Whether that looks like hospice care, non-medical support through Transitions, or a provider who comes to your door, the goal has never changed. We meet people where they already are.

September is World Alzheimer's Month.

If you are caring for someone with dementia, you already know it can be one of the most tender and tiring roles there is.

Here are a few things that tend to help. Keep daily routines steady, offer simple choices rather than open questions, and meet your loved one in their reality instead of correcting it. And be gentle with yourself, because your care matters too.

You are not walking this road alone. Our team is here for families across Central Oregon.

Every tree at Festival of Trees is thoughtfully designed and built by someone in this community. They are truly works of art. In this video, Stephanie is looking for decorators for this year. No experience required! You just have to want to do something that can make a lasting impact right here in Central Oregon.

This week is Falls Prevention Awareness Week, and for older adults, a single fall at home can change everything. That is one of the reasons in-home care matters so much. When Taylor and Helan visit a patient at home, they see the whole picture that an office visit can miss, from medications and mobility to the small details of the space where someone actually lives. Caring for people where they are is one of the best ways to help them stay safe, steady, and independent at home.

September is Healthy Aging Month, and for many older adults here in Central Oregon, aging well simply means staying in the home they love. That is what our In-Home Primary Care team was built for.

Taylor and Helan travel directly to patients across Deschutes, Crook, and Jefferson counties, giving each visit a full 45 to 60 unhurried minutes and, if needed, caring for the family caregiver too.

The program is billable to Medicare, Medicaid, and private insurance, with no membership or enrollment fees.
